Healthcare at Home

Presbyterian Healthcare at Home includes all of our care services for patients who either need or are more comfortable receiving care wherever they call home in New Mexico. These services include home health, hospice care, palliative care and respite care. Our team works closely with the patient’s healthcare provider to monitor your progress and health concerns.

What areas of New Mexico do you serve?

We have service areas throughout most of New Mexico. Patients can be seen in their homes in the Albuquerque metro area, as well as in Española, Santa Fe, Clovis, Tucumcari and Socorro and surrounding areas. We also offer inpatient hospice services in Albuquerque and telemedicine (virtual) palliative care services in select locations. What services does Healthcare at Home provide?

We provide the following services:

• Home Health: Includes medical services provided in the home setting to treat a chronic health problem or illness, or to help you recover from a surgery, illness or injury.

• Hospice: Provides care that focuses on quality of life and comfort for patients with serious, end-of-life illnesses.

• Palliative Care: Focuses on helping people with serious illness or chronic conditions manage their symptoms and improve their quality of life.

Respite Care: A short-term relief for primary caregivers, providing care for the patient while the caregiver needs time to look after their own needs.

Do I qualify for Healthcare at Home?
