Palliative Care
Palliative care focuses on medical care that improves quality of life and relieves physical and emotional symptoms for people with serious, complex or terminal conditions. Palliative care is appropriate for anyone of any age, or at any stage of a serious illness. Although palliative care can be an important part of care at the end of a person's life, it is not limited to that. In fact, our palliative care team works closely with other healthcare providers in order to alleviate physical symptoms, integrate spiritual and emotional well-being for patients and their caregivers, and provide coordination of care between treatment settings such as your home and the hospital.
Palliative care can be provided alongside curative treatment, or as the main goal of treatment itself.
Palliative care can be provided in a variety of locations including the hospital, outpatient clinics, skilled nursing facilities, long-term care facilities, and the home environment. Our team includes physicians, nurses, and social workers. We collaborate with a person's other physicians, dieticians, physical and occupational therapists, chaplains and psychologists. Palliative care is sometimes used with other Healthcare at Home services, such as home health and hospice.

Palliative care is provided by members of our trained team including medical director, physicians, nurses, social workers, therapists, home health aides and counselors.
CLINICS & FACILITIES
Palliative medicine in-home services are offered through Presbyterian Healthcare at Home, as well as in person at a variety of Presbyterian Medical Group clinics in Albuquerque and Rio Rancho. Palliative medicine may also be available in some locations via telemedicine services.
