Presbyterian Healthcare at Home provides care services for patients who either need or are more comfortable receiving care at their home. These services include home health, and hospice, palliative, and respite care. Our Albuquerque-based team serves the Albuquerque metro area, as well as Rio Rancho. This location also provides inpatient hospice and respite care at Presbyterian Kaseman Hospital if needed.
Our multidisciplinary hospice team of skilled nurses, social workers, home health aides, chaplains, volunteers and our medical director provide physical, emotional and spiritual support to patients and their families by creating a plan to help patients maintain their dignity, comfort and quality of life when curative measures are no longer an option. Some of the services we offer include:
Care coordination
Pain and symptom management
Nutritional consults
Physical or occupational therapy
Emotional and spiritual support
Short-term inpatient care for symptoms management if needed
Services provided by Presbyterian Healthcare at Home. Some services may use independent community healthcare providers.
