Presbyterian Healthcare at Home provides care services for patients who either need or are more comfortable receiving care at their home. These services include home health, and hospice, palliative, and respite care. Our Socorro-based team serves Socorro and surrounding areas.
Our multidisciplinary team of skilled nurses, social workers, home health aides, chaplains, volunteers and our medical director provide physical, emotional and spiritual support to patients and their families by creating a plan to help patients maintain their dignity, comfort and quality of life when curative measures are no longer an option.
