Presbyterian Healthcare at Home provides care services for patients who either need or are more comfortable receiving care at their home. These services include hospice care, palliative care in certain locations, and respite care. Our Santa Fe-based team serves the Santa Fe and Espanola area. This location also provides inpatient hospice and respite care at Santa Fe Medical Center and Presbyterian Espanola Hospital if needed.
Our multidisciplinary hospice team of skilled nurses, social workers, home health aides, chaplains, volunteers and our medical director provide physical, emotional and spiritual support to patients and their families by creating a plan to help patients maintain their dignity, comfort and quality of life when curative measures are no longer an option.
