Changes to Presbyterian UltraFlex (HMO-POS) Plan
We’re determined to keep your Medicare Advantage coverage strong.
Effective December 31, 2025, Presbyterian UltraFlex (HMO-POS) Medicare Advantage plan will be discontinued.
You will need to choose a new plan before December 31, 2025, that fits your healthcare needs.
If you do not choose a plan before then, you will be enrolled into Original Medicare (Parts A & B) only on January 1, 2026. Original Medicare does not include Part D drug coverage, dental, vision, hearing, and other added benefits you may be used to.

We offer a variety of Presbyterian Medicare Advantage plan options. Our Presbyterian Senior Care HMO Plan 2 is the most similar to UltraFlex. It is a $0 premium plan with prescription coverage, $0 primary care visits, the same dental coverage options, and vision benefits. Not all plan types are offered in all counties.
