Welcome to Your 2010 Presbyterian Health Plan

As an employee of TriCore Reference Laboratories, you have two benefit plans available to you. If you live in the state of New Mexico, you may select the Smart Care 750. If you live outside the state of New Mexico, you may select the Indemnity plan.

The following Schedules of Benefits are summaries that describe the copayment amounts that apply to specific types of services. Some benefits require pre-authorization by PHP. Benefits may have limits and certain services are excluded altogether.
